E-mail Medical Certificate applications (CG 719K or K/E) to MEDAIP@uscg.mil. Put LAST NAMEFIRST NAMEMARINER REFERENCE NUMBER in the subject line of the email to MEDAIP@uscg.mil. Medical applications (719K) should be submitted as ONE PDF.
(1) Every five years, in ance with the medical certificate requirements in 10.301(b), 10.302(a), and 10.304(d) of this chapter, the results of the most recent physical examination must be submitted to the Coast Guard.
An MMC is valid for a term of 5 years. Adding an endorsement during this period does not change the expiration date. The only time the expiration date will change is when a mariner applies for and meets all renewal requirements. Once an MMC has expired, a mariner may not serve under the authority of that credential.
To formally request a waiver, you must submit a waiver application to the NMC. The application should include: A written request for a waiver. All supporting medical documentation.
