Click ‘Get Form’ to open the account redemption form in the editor.
Begin by entering your Invesco Account Number and Registration Information. Ensure you provide your SSN or TIN, account registration name, primary and alternate phone numbers, and date of birth clearly in block capital letters.
In the Redemption Instructions section, select whether you want to redeem the entire account or specific funds. If choosing specific funds, list the fund number, fund name, percentage, or dollar/share amount for each fund.
If applicable, choose your cost basis method from the options provided. Remember to attach any additional pages if necessary for specific lot identification.
Fill out your Payment Options by indicating how you would like to receive your proceeds—either by check or bank transfer. Provide all required banking information if opting for ACH transfer.
Sign and date the Authorization and Signature section. Ensure that all signatures are guaranteed if required based on the transaction amount or recipient.
Finally, follow the Mailing Instructions to send your completed form to Invesco Investment Services using either direct mail or overnight mail options.
