2. Begin by entering your Invesco Account Number and Registration Information. Ensure you provide your SSN or TIN, account registration name, primary phone number, email address, and date of birth. Use blue or black ink and print clearly in block capital letters.
3. In the Redemption Instructions section, select whether you want to redeem the entire account or specific funds. If choosing specific funds, list the fund numbers, names, and amounts accordingly.
4. For Payment Options, choose between receiving a check or transferring funds to your bank account. If opting for a check, specify if it should be made payable to someone other than the account registration.
5. Complete the Authorization and Signature section by signing and dating the form. Ensure all required signatures are included based on your transaction details.
6. Finally, follow the mailing instructions provided at the end of the form to send it to Invesco Investment Services.
