5. For alternate AM transportation, choose an existing bus stop or circle 'Other' if applicable. Indicate your preference for AM Rec, JCC, or Little Lukes and select the days of the week needed.
6. Repeat step 5 for alternate PM transportation needs.
7. Indicate parent drop-off preferences by circling the appropriate days for both AM and PM options.
