In Section 1(b), enter the IRS Identification Number, followed by the SEC File Number in Section 1(c). If you do not have these numbers, contact the issuer for assistance.
Complete the issuer's address in Section 1(d), including street, city, state, and zip code. Don’t forget to add a telephone number in Section 1(e).
Proceed to Section 2(a) and input the name of the person for whose account the securities are being sold. Specify their relationship to the issuer in Section 2(b).
Fill out their address details in Section 2(c), ensuring all information is correct and complete.
Continue with Sections related to securities details, including title, number of shares, and approximate sale date. This information is vital for compliance.
Finally, review all entries for accuracy before signing at the bottom of the form. Ensure that at least one copy is manually signed.
