CHECKLIST OF REQUIREMENTSWHERE TO SECURE Affidavit of Admission of Paternity/Acknowledgement found at the back of the COLB or in separate affidavit duly registered in the MCRO in four copies. MCR Office and/or Notary Public7 more rows
By signing this Certificate of Parentage you are legally establishing your childs paternity. Paternity means fatherhood. For parents who are not married to each other, paternity may be established legally only by signing this form or by going to court.
The fathers name will appear on the birth certificate, and the father must consent to the name of the child before the childs name appears on the birth certificate. A father signing just the birth certificate, without signing a Paternity Acknowledgement Affidavit, does not constitute paternity.
