Proceed to Step 2 and ensure all fees are paid. If you are a current member, confirm that there are no outstanding fees. For lapsed or expired certifications, calculate any necessary fees and include them with your application.
Complete the Recertification Application on page 4 of the booklet. Fill in your personal information accurately and check off whether you meet the requirements for recertification.
Finally, review your application for completeness before mailing it to BONENT at the provided address. Remember that faxed or photocopied applications will not be accepted.
