2. Begin by entering the child's current information as displayed on the STAC Online (EFRT) System. Fill in the child's last name, middle initial, first name, STAC ID, gender, date of birth, and race-ethnicity.
3. Next, indicate the changes you wish to make. For each field that requires an update (first name, middle initial, last name, date of birth, gender, race-ethnicity), enter the new information clearly.
4. Provide a reason for the change by selecting from the options available or writing 'Other' if applicable. If relevant, include details about adoption or gender identification changes.
5. Ensure that the form is signed by an authorized representative such as a district superintendent or municipality representative. Include their printed name, title, email address, phone number, and date.
6. Once completed, scan and upload the form to SED File Transfer Manager (FTM) ‘inbasket’. Remember not to attach completed forms to emails; instead, email OMSSTAC@nysed.gov with your FTM location and filename.
