1. Click ‘Get Form’ to open it in the editor.
2. Begin by entering the child's current information as displayed on the STAC Online (EFRT) System. Fill in the child's last name, first name, middle initial, and STAC ID accurately.
3. Indicate the child's gender identity by selecting from Male, Female, or Non-Binary options provided.
4. Input the child's date of birth and race-ethnicity as currently listed on EFRT.
5. In the 'Change' sections, specify any updates needed for the child’s first name, middle initial, last name, date of birth, gender identity, and race-ethnicity. Ensure all changes are clearly marked.
6. Select a reason for change from the options provided or specify another reason if applicable.
7. Complete the certification section by signing and providing your title, email address, and phone number. Make sure to verify all information against legal documentation before submission.
8. Once completed, scan and upload your form to SED File Transfer Manager (FTM) ‘inbasket’ and email OMSSTAC@nysed.gov with details of your submission.
