Begin with Section I, where you will provide details about the first child. Fill in the child's name, date of birth, and address at the time of removal. Ensure you include their U.S. Social Security Number and place of birth.
Next, complete Section II for the applicant. Enter your name, relationship to the child, date of birth, citizenships, and contact information including your current address and email.
In Section III, provide information about the person alleged to have wrongfully removed or retained the child. Include their name, relationship to the child, and any known aliases.
Continue filling out Sections IV through IX by providing additional details about other children involved, circumstances of wrongful removal, proposed arrangements for return travel, and any other relevant information.
Finally, sign the application in blue ink and ensure all required documents are attached before submitting.
