1. Click ‘Get Form’ to open the prorate account checklist in the editor.
2. Begin by entering your Full Name and Business Name (if applicable) in the designated fields. Ensure that all information is printed clearly.
3. Provide your Daytime phone number and Prorate Account number in the respective fields for easy communication.
4. Review the checklist of required items before starting the application process. Make sure you have your USDOT Number, FEIN, HVUT Form 2290, proof of vehicle ownership, and lease agreement if needed.
5. Complete the five forms listed: IRP Minnesota Presence Affidavit, New Prorate Applicant Questionnaire, Estimated Mileage Worksheet, Minnesota IRP Renewal/Supplement Application, and IFTA License and Decals Application.
6. Once completed, submit this checklist along with all forms either by mail or in person to the specified address or fax it to (651) 215-0027.
7. If approved, visit our office or a Prorate Deputy Registrar’s office with original documents for final processing.
