The health professional will advise the DWP which descriptor applies to you for each activity. For example, there are 6 descriptors for Dressing and undressing, ranging from Can dress and undress unaided to Cannot dress or undress at all. Each descriptor carries a points score ranging from 0 to 12.

4 points. Needs assistance to be able to manage a therapeutic source to take nutrition. 6 points. Cannot convey food and drink to their mouth and needs another person to do so.
How to score points on PIP form?
For example, if you need help from another person to wash your hair, you get 2 points. If you need help to get into the shower or bath you get 3 points, etc. You only score one set of points from each activity (for example, if you meet two descriptors in washing and bathing, you cannot receive points for both.
