9. Complete Box 5 by summing amounts from previous boxes, then calculate 2% for collection costs in Box 6.
10. Subtract Box 6 from Box 5 to find TOTAL PRIMACY FEES PAID TO DNR, which is what you will submit.
11. Sign and date at the bottom of the report before mailing it along with your payment to DNR.
