Begin by filling out items 1 through 7, and item 18. Ensure you include 'completed by' and 'phone number' on every DDP-1.
For item 1, select the purpose of the form. This could be updating demographic data or indicating a change in residence.
In item 2, enter the TABS ID if known. If not, proceed to fill in the person's name (item 3) using their birth certificate or Medicaid card for accuracy.
Complete item 5 with the date of birth, followed by item 6 for county of residence. Make sure to provide accurate residential address details in item 11.
Indicate the individual’s residence type in item 12 and fill out items related to disabilities (item 16) as applicable.
Finally, ensure that item 18 is completed with your name and phone number before saving or submitting the form.
