1. Click ‘Get Form’ to open the Badge Return/Replacement Form in the editor.
2. Begin by filling out Section 1, which includes the date, your name, and employee ID number. Ensure all information is accurate for processing.
3. If you are requesting a replacement for a lost badge, complete Section 2. Indicate whether it’s a first-time replacement or a subsequent one and authorize the deduction from your paycheck.
4. For damaged badges, Section 3 requires you to return the damaged badge along with this form. Note that replacements will be sent via interoffice mail within 3-5 business days.
5. If there has been a name change, complete Section 4. Remember that this request must be processed through Employee Self Service before issuing a new badge.
6. In Section 5, indicate if you are returning your badge because you no longer work with the district or if it is a lost badge return.
7. Finally, sign in Section 6 and provide your campus/department details along with any specific recipient for the Pony ID Badge.
