  19. 'Check' boxes for attestations regarding meat and poultry products compliance in Blocks 18 and 19 respectively.
  20. Sign and print certifying official’s name and title along with district number in Block 20 using a color other than black ink.
  21. Finally, enter the date signed in Block 21.
