How to use or fill out vahps with our platform
- Click ‘Get Form’ to open the vahps application in the editor.
- Begin by filling out the 'Shipper' section. Enter your ID number (VAHPS ID, FEIN, or SSN) and indicate if this is a new shipper location or an address change.
- Provide the contact details including the contact person's name, company name, telephone number, mailing address, fax number, email address, city, state, zip code, VA jurisdiction, and country.
- In the 'Vehicle Information' section, enter the vehicle license plate number, issuing state, and VIN/serial number (last 5 digits).
- Select the appropriate exempt permit type and specify the quantity of permits required.
- Complete the certification section by printing your name, signing it, entering the date signed, and providing any necessary PSC information if applicable.
