Provide the contact details including the contact person's name, company name, telephone number, mailing address, fax number, email address, city, state, zip code, VA jurisdiction, and country.
In the 'Vehicle Information' section, enter the vehicle license plate number, issuing state, and VIN/serial number (last 5 digits).
Select the appropriate exempt permit type and specify the quantity of permits required.
Complete the certification section by printing your name, signing it, entering the date signed, and providing any necessary PSC information if applicable.
