2. Begin by entering your Finance No. and SFS Customer ID in the designated fields. If this is your first order, indicate that in the box provided.
3. Fill in the CPU Contact Person's name, along with the Site ID, CPU Name, and CPU Address. Ensure all information is accurate for smooth processing.
4. Provide your City, State, ZIP + 4 code, Phone Number, Email, and Fax number. All fields are required; please type or print clearly.
5. Select your payment method: either CHECK/MONEY ORDER or ACH Debit. If using ACH Debit, fax the completed form to the specified number.
6. List each stamp request under 'Single Orders', including Description, Item #, Format (Pane, Coil, Booklet), Quantity, and Total $ for each item.
