  11. Provide Country of Origin for customs purposes.
  12. In Description of Goods, detail what is being shipped including package type, marks, numbers, and a clear description of each item.
  13. Indicate Quantity for each item listed in your description field.
  14. Fill in Unit Cost for each item based on your pricing structure.
  15. Calculate Total Cost by multiplying Quantity by Unit Cost for each item listed.
  16. If goods are not sold, state reason for exportation clearly.
  17. List any Additional Costs that may apply such as insurance or freight charges.
  18. (Optional) Complete NAFTA Certification if applicable and sign where indicated.
  19. Finally, ensure you calculate and enter Invoice Total before signing and dating your document at the bottom.
