8. Specify any assistance needed with daily activities (ADLs/IADLs) by checking off relevant boxes.
9. If there are current services being utilized by the client, indicate them along with provider names where possible.
10. Once all sections are completed, review your entries for accuracy before submitting via email or fax as indicated at the top of the form.
