1. Click ‘Get Form’ to open the ppd form in the editor.
2. Begin by entering the Employee Name and Date of Birth in the designated fields. Ensure accuracy for proper identification.
3. Fill in the Department and Supervisor's name, along with their contact information (Phone #, Pager, or email address) for easy communication.
4. For the PPD (TB) Skin Test Placement section, input the Date Test Applied and the name of the person who applied it in the respective fields.
5. Select the PPD solution manufacturer and its expiration date. Choose the test type by circling either '2 Step', 'Annual', or 'Post Exposure'.
6. Indicate where the test was applied (Left, Right, or Forearm) and record the Lot Number.
7. After 48-72 hours, read and record the results of the skin test in millimeters under PPD (TB) Skin Test Results. Mark whether it is Neg or Pos.
8. Finally, fax results to 874-3410 as instructed and reach out to Employee Health if you have any questions.
