The DD Form 1172-2 shall be used to apply for issuance of a DD Form 3208, Uniformed Services Identification (USID) Card (Armed Forces of the United States Geneva Conventions Identification Card, Sponsor Identification and Privilege Card, and Dependent Identification and Privilege Card), and a Common Access Card (CAC),
Sign into milConnect, open the Correspondence/Documentation menu, choose the eCorrespondence page, and then select its Proof of Coverage tab. Follow the directions to generate and print the letter. You can also contact the DMDC/DEERS Support Office.
Uniformed Services ID (USID) card Use the RAPIDS self-service ID office online to report a lost or stolen ID. If you are a dependent, you will also need to complete a Form DD 1172-2. Submit this form to the ID office when you request a replacement card.
Go to a RAPIDS site with your completed DD Form 1172-2 and two forms of identification, including a state or federal government photo ID. Dependents will need a Social Security number or tax ID number if they are U.S. citizens or U.S. persons.
What documents are required for renewing ID card? Two forms of valid identification (one must include photo). If sponsor is not present, the family member must provide an Application for an Identification and Privilege Card (DD Form 1172-2) signed by sponsor and notarized or a valid Power of Attorney.
