Next, provide the vehicle information including the Vehicle Identification Number (VIN), operator license number, year, make, and registration plate number.
If applicable, check any boxes for changes to existing IID records such as moving an IID from another vehicle or changing vendors.
Proceed to sign the Operator Certification section. Make sure to read the statements carefully before signing and dating the form.
If you are not the vehicle owner, complete Part 2 by having the registered owner fill in their information and sign.
Finally, in Part 3, ensure that an approved installer completes their section after installation. This includes entering IID type and model along with their signature.
