Click ‘Get Form’ to open the DR-1S application in the editor.
Begin by entering your Business Partner Number, which can be found on your sales and use tax Certificate of Registration (Form DR-11).
Provide your Business Identification Number, either the Federal Employer Identification Number (FEIN) or Social Security Number (SSN) of the business owner.
Select your Business Structure and Ownership by checking the appropriate box for Sole Proprietorship, Partnership, Corporation, or Limited Liability Company.
Fill in the Legal Name of your business entity and any Trade Name if applicable. Ensure all addresses are complete and accurate.
List all owners and controlling members by providing their full names, titles, SSNs, addresses, and telephone numbers. Use additional pages if necessary.
For each location you are registering, enter the Sales Tax Certificate Number and provide a physical address. Remember to include a $6 fee for each location.
Finally, sign the application where indicated and ensure that it is signed by an authorized individual from your business entity.
