2. In Section I, select the type of unit by checking the appropriate box: Manufactured Home, Mobilehome, Commercial Modular, Truck Camper, or Floating Home. Enter the Decal (License) Number(s), Trade Name, and Serial No.(s) as required.
3. Move to Section II for Trust Information. Confirm that the unit is placed into a trust by signing as trustee(s). Fill in the Declaration of Trust date and print the name of the trust as it should appear on title.

5. In Section III, certify under penalty of perjury that all information provided is true and correct. Complete the execution details including date, city, state, zip code, and signatures of each trustee.
