Transferring Immunization Records from Another State to Florida. The Florida Certification of Immunization, also known as the \u201cBlue Card\u201d or \u201cDH 680 form,\u201d is required for each child entering a Florida school for the first time. This form must be completed by a physician or a Florida county health department.
The State of Georgia requires children to have certain immunizations to attend public school. This includes all children attending day care, kindergarteners, rising 7th graders, rising 11th graders, and those transferring into the state or from a home/private school.
In Georgia, all children born on or after January 1, 2002, who are attending seventh grade, and children who are new entrants into a Georgia school in grades eight through twelve, must have received one dose of Tdap vaccine and one dose of meningococcal conjugate vaccine to fulfill immunization requirements.
These requirements include documented proof of immunity to measles, mumps, and rubella, varicella (chicken pox), tetanus, diphtheria, pertussis and hepatitis B, prior to registration at any college or university within the University System.
Submit any immunization records to either immunizations@gsu.edu with panther ID# written on the document or to the student portal at health.gsu.edu. Other Holds: If your registration hold is not immunizations related, please contact GSU Admissions office at 404-413-2000 for further help.
