Next, move to the Carrier Information section. Input the carrier/DBA name and legal name if different. Provide the federal ID number and street address along with the city.
Indicate whether there were any motor vehicle accidents in the past 12 months by selecting 'YES' or 'NO'. If 'YES', complete the Accident Information section detailing dates, locations, and descriptions of damages.
Proceed to record any traffic violations or crimes in the Record of Convictions section if applicable. Fill in dates of violations and convictions along with charges and court locations.
Complete both Driver Certification and Carrier Certification sections by signing and dating where required.
