2. Begin by filling in your Policy No. and e-Insurance Account No. if applicable, especially for demat customers.
3. Enter the Name of the Policyholder/Beneficiary and provide a valid Email ID.
4. Fill in your Contact Numbers in the specified format, ensuring you include country and city codes.
5. For Unit Linked Young Star or Children's plans, provide beneficiary’s/Appointee’s account details if they are a major.
6. Complete the Bank Account details including Account Holder Name, Bank Name & Branch, and select your Account Type (Savings, Current, NRE, NRO).
7. Input the IFSC Code as indicated on your cheque leaf.
8. Attach a cancelled personalized cheque or a recent bank statement/passbook copy as required.
9. Review all information for accuracy before signing and dating the declaration section at the bottom of the form.
