Airlines may require: (1) a U.S. DOT form attesting to the animals health, behavior, and training; and. (2) a U.S. DOT form attesting that the animal can either not relieve itself or can relieve itself in a sanitary manner, if the animal will be on a flight that is 8 or more hours.
The ADA requires that you have written documentation from your healthcare provider stating that you have and are being treated for a disability requiring assistance from a Service Dog. In addition, the work the Service Dog has been trained to do must specifically relate to your condition.
Service Animal Request. You must complete and submit the U.S. DOT Service Animal Air Travel Form prior to your flight attesting to the animals health, training, and behavior.
