1. Click ‘Get Form’ to open the orion form in the editor.
2. Begin by entering the Consumer Name and Employee Name at the top of the form. This information is essential for identifying the service recipient and provider.
3. Select the appropriate County and Month for which you are reporting hours. Ensure this matches your service period.
4. In the Shift Start and Shift End columns, record your working hours, specifying am or pm. Repeat this for each shift worked.
5. For each shift, indicate the Service Type using the corresponding three-letter code (REG, RESP, HOM, CHO) as outlined in the Service Type Guide.
6. Calculate and enter Total Hours worked for each day in the designated column. Ensure that no more than 40 hours are reported per week.
7. Complete any additional fields such as Comments if necessary, then ensure both Employee and Employer signatures are provided before submission.
