Begin by filling out the CUSTOMER INFORMATION section. Enter your name or company name, DTE Energy account number, and the last four digits of your Social Security number or Federal Tax ID number, ensuring all required fields are completed.
If you are a business customer, provide the billing contact person's name, phone number, fax number, email address, and billing address. Make sure to include city, state, and zip code.
In the INFORMATION COMMON TO ALL SITES section, fill in the site contact person's details and development type. Select from options like single-family home or condominium as applicable.
Complete the service city/township/village and county information. If known, include the DTE Energy service planner's name and phone number.
For individual site addresses, enter house numbers and street names for each location. Specify service type and construction meter type as needed.
