2. Begin by filling out the Applicant Information section. Enter the title, last name, first name, middle name, Social Security Number or GaPSC Certification ID, and date of birth in the specified format.
3. In the Certification Official Section, check the appropriate box to certify whether the applicant has completed all requirements for certification. Provide degree information by selecting if a degree was awarded or if it was certification only.
4. Complete the Student Teaching Internship Information by indicating where student teaching was completed and providing details about the supervising teacher if applicable.
5. Fill in Program Information including start and end dates of the program and select accreditation/approval status as required.
6. Finally, ensure that all sections are signed by the certification officer and include their contact information before submitting.
