The DD Form 2792 is completed to identify a family member with special medical needs. There is a Certification Section on page 3 that should be signed AFTER the entire form is completed by medical provider(s) and the form has been reviewed for completeness and accuracy.
This form is used to identify the custodian of an unmarried minor child, incapacitated minor child, or child at least age 18 but under age 22 attending school full time, when a claim for annuity payments is made on behalf of the child.
Service members are given the option of accepting the short form edited Member 1, long form unedited Member 4 or both copies upon separation. The most important copy of the DD 214 for the individual is the long form copy.
