3. For the Birthdate, input the month, day, and year using MMDDYY format. This is conditionally required when billing charges without a recipient number.
4. Enter the Recipient No., a nine-digit number from the MediPlan Card, ensuring no punctuation or spaces are included.
5. In section 23e, specify the Type of Service by entering the corresponding code for medical care or surgery. Remember that only one type of service can be included per claim.
6. Complete all required fields such as Date of Service and Provider Charges accurately to avoid claim rejection.
7. Finally, ensure that you sign and date the form in black or dark blue ink before submitting it through our platform.
