1. Click ‘Get Form’ to open the gem claim form in the editor.
2. Begin by filling out the 'Member Details' section. Enter your GEM membership number, name, address, email for confirmation of refund, and telephone number.
3. Next, provide your vehicle details including make & model, registration number, and date first registered. Indicate if you are the registered owner and provide details of the driver at the time of breakdown.
4. In the 'Breakdown Details' section, specify where you were traveling from and to, the purpose of your journey, place of breakdown, date and time of breakdown, cause of breakdown, and number of occupants.
5. Complete the 'Details of Claim' section by entering costs for roadside repairs, recovery services, alternative travel means, and emergency overnight accommodation if applicable.
6. Once all sections are filled out accurately, click ‘Save & Lock’ to protect your data from further editing. Finally, send your completed form via email to reclaim@motoringassist.com along with any required receipts.
