3. Select the number of years of practical experience you are claiming by ticking one box (3, 4, or 5 years) under the Employment Summary section.
4. In Part A, list your employers chronologically, starting with your current employment. For each employer, indicate the training year and total number of working days acquired.
5. Fill out the Practical Experience Components section by detailing your competencies achieved during each training year. Make sure to tick off units as you complete them.
6. At the end of the form, ensure you sign and date it. If applicable, have your Authorized Supervisor or Counselor sign as well to validate your experience.
