Although most patients in hospice are discharged due to death, live discharges do occur and are reviewed for each survey. The reasons for live discharge can include extended prognosis, leaving the service area, revocation, transfer to another hospice and discharge for cause.
(1) The patient moves out of the hospices service area or transfers to another hospice; (2) The hospice determines that the patient is no longer terminally ill; or.
Hospice discharge may occur if: The patients condition stabilizes or improves docHubly. A new treatment becomes available that changes the prognosis. The patient no longer meets Medicare or insurance eligibility criteria for hospice.
The hospital will discharge you if you no longer need to be there for your care. But this may not mean that you are fully healed or recovered. You may have a medical condition that still needs attention and care.
The hospice discharge summary must include: A summary of the patients stay including treatments, symptoms and pain management. The patients current plan of care. The patients latest physician orders.
