Church funds request form 2026

1. Click ‘Get Form’ to open the church funds request form in the editor.
2. Begin by filling out the GENERAL INFORMATION section. Enter your department, name of ministry/team, ministry leader, and your name in the respective fields.
3. Select the requesting dates by circling one of the options provided: Jan 1 - Mar 31, Apr 1 - Jun 30, Jul 1 - Sept 30, or Oct 1 - Dec 31.
4. In the REQUEST INFORMATION section, specify the request type by selecting one of the options: Purchase Request, Advanced Funds, Reimbursement, Invoice, or Credit Card.
5. Fill in the amount requested and provide a detailed purpose for the funds. Include payee information and a description of items needed along with their quantity and total amount.
6. Indicate the date needed for these funds and add any additional comments that may be relevant to your request.
7. Once completed, review all entries for accuracy before submitting it to your Department Head for processing.
