3. In the 'Type of School' section, check the appropriate box that corresponds to your institution type. Provide details for both a primary contact person and an alternate contact person.
4. Next, move to 'Field Trip Information.' Fill in the group leader’s name and grade level. Indicate your preferred dates for the trip by listing your first, second, and third choices.
5. For 'Reservation Information,' specify whether you want to include an IMAX film or just visit the BODY WORLDS 3 exhibit. Fill in details such as film title, time, number of students and chaperones.
6. Complete the 'Payment Method' section by selecting your payment option and providing necessary card details if applicable. Ensure you sign at the bottom to acknowledge understanding of cancellation policies.
