8. Specify whether you are a microchip implanting or scanning person or involved in tattooing. Fill out your contact information including telephone number.
9. Complete any veterinary practice numbers required for tattooing or microchipping and provide kennel license information if applicable.
10. Finally, sign and date where indicated for both the person implanting/scanning and the dog owner. Ensure that this form is returned to the county treasurer within 30 days of receipt.
