Four Component Model of Decisional Capacity. Capacity for healthcare is generally defined in terms of four dimensions or criteria: (a) Understanding, (b) Appreciation, (c) Reasoning, and (d) Expression of a Choice (Grisso Appelbaum, 1998a; Roth, Meisel, Lidz, 1977).
With the fundamental purpose clear, stakeholders should then brainstorm a list of objectives what they need and want from the ideal choice aligning around what I think of as the Four Rs of rational decision-making: Results Resources Restrictions and Risks.
Decision-making capacity requires the presence of 4 abilities: (1) appreciation of ones situation and the consequence of ones choices, (2) understanding of the information required to make the choice, (3) reasoning about the risks and benefits of choices, and (4) communication of ones choice.
