3. Fill in your student information. Indicate whether you were enrolled in undergraduate or graduate courses, and provide the dates you attended.
4. Enter your full name, Student ID number or last four digits of your SSN, and any different name used while attending. Don’t forget to include your date of birth.
5. Complete your home address, including city, state, and zip code. Provide your home, cell, and work phone numbers along with your email address.
6. If applicable, check 'Yes' to update your information and select the type of email you prefer for correspondence.
7. Fill out the mailing instructions for where you want the transcripts sent, including company name and attention line if necessary.
8. Sign and date the form before submitting it via mail, fax, or email to transcripts@mcdaniel.edu. Ensure payment is made online if required.
