3. Select the appropriate test fee based on your test date. Choose either $317 for tests prior to 1 August 2011 or $330 for tests after this date.

5. If paying by credit card, fill in the Card type (Visa/Mastercard), Card number, CVV, and Expiry date (mm/yy) in the provided fields.
6. Complete the Payment Authority section by signing and providing your full name as it appears on the card, along with the date of signature and your email address.
