8. Review the requirements section and ensure no courses for the minor are taken P/F. If substitutions are needed, list them under Substitution I and II.
9. Sign and date where indicated for Student Signature, Department Head Signature, and Advisor Signature.
10. Once completed, return the signed form to the Office of the Registrar as instructed.
