4. After the appointment, Section II must be filled out by the medical provider. This includes reporting times and any necessary restrictions on work duties.
5. Finally, upon returning to the work center, provide a copy of the completed permit to the Supervisor and send copies to CHRO-E and MCIEAST-MCB CAMLEJ Safety Department.
