An oral report is made to the Statewide Central Register of Child Abuse and Maltreatment (SCR) when there is reasonable cause to suspect child abuse or maltreatment. If the SCR accepts the report, the reporter must then complete and submit the LDSS 2221A form within five business days of the oral report.
Within 48 hours of the mandated reporters verbal report to the SCR Mandated Reporter hotline, the mandated reporter must complete and submit form LDSS- 2221A to the local department of social services Child Protective Services unit.
Reports of suspected child abuse or maltreatment shall be made immediately by telephone and in writing within 48 hours after such oral report. Submit the written paper copy of the LDSS-2221A form originally signed to: the Local County Department of Social Services (LDSS) where the abused/maltreated child resides.
