Who can Participate? DC residents with IDD who are at least 18 years of age are eligible to participate in My Life, My Way. Participants must: live in their natural home, be eligible to receive Medicaid, and be enrolled in the Individual and Family Support (IFS) wavier.
If you would like assistance with the application process, please call us at 202-724- 5626. Your Medicaid provider will complete this form to initiate a face-to- face assessment by Liberty Healthcare. This is your application to the EPD Waiver program.

Instead of going to a nursing home, you can stay in your own home and community. All while getting the support you need to live your life and reach your goals.
The AD Waiver offers an array of services to support people in their homes. It caters to individuals of all ages who are eligible for Medicaid and have complex medical needs, want to live at home, and can be served safely at home.
DD waiver is better for my autistic boys because it captures more of their behavior issues while Cadi is a smaller budget because it doesnt cover the behavior issues that much. DD does have a stricter criteria for approval as well.
