A plus (+) sign in front of the number means you are farsighted, and a minus (-) sign means you are nearsighted. These numbers represent diopters, the unit used to measure the correction, or focusing power, of the lens your eye requires. Diopter is often abbreviated D.
4 years old: the critical line to pass screening is the 20/40 (10/20) line. 5 years and older: the critical line to pass screening is 20/32 (10/16) or 20/30 (10/15) line depending on the chart used. Must be able to identify the majority of the 20/50 (10/25) line with each eye.
Record your visual acuity for each eye by noting the line for which you correctly identified either: a) More than half the letters on that line, but not all of them. b) All letters on that line, plus a few letters (less than half) on the next line.
Your visual acuity score is measured by a fraction. Perfect vision yields a score of 20/20, which means the viewer can see the same letters at 20 feet away what another normal viewer could also see at 20 feet. If your vision is less than perfect, you might receive a score of 20/40 or 20/50.
Sphere (SPH) It measures how long or short-sighted you are. It is followed by a number which is the unit used to measure the correction (dioptres). Generally, the further the number is from 0, the stronger the correction you need. If the SPH number has a minus sign (), it means that you are short-sighted.

Results of a routine eye exam are normal when the eye doctor finds you have: 20/20 (6/6) (normal) vision. Ability to identify different colors. Full visual field. Proper eye muscle coordination. Normal eye pressure. Normal eye structures (cornea, iris, lens)
Visual acuity is expressed as a fraction, such as 20/20. Having 20/20 vision means that your visual acuity at 20 feet away from an object is normal. If you have 20/40 vision, for example, that means you need to be 20 feet away to see an object that people can normally see from 40 feet away.
Normal Results Visual acuity is expressed as a fraction. The top number refers to the distance you stand from the chart. This is often 20 feet (6 meters). The bottom number indicates the distance at which a person with normal eyesight could read the same line you correctly read.
A plus number indicates long-sightedness, so objects up close appear more blurred or close vision is more tiring on the eyes. The sphere value moves up and down in 0.25 increments, anything smaller is imperceptible. Patients will commonly fall within the -25.00 or +25.00 range and the majority between +4.00 or -4.00.
For example, 20/20 (6/6) is considered normal. 20/40 (6/12) indicates that the line you correctly read at 20 feet (6 meters) away can be read by a person with normal vision from 40 feet (12 meters) away.
