The Home and Community-Based Services (HCBS) Waiver programs serve persons with disabilities who live at home or in non-medical facilities (like Assisted Living or Board and Care facilities) instead of a nursing home (a.k.a. long-term care facility).
HCBS programs address the needs of people with functional limitations who need assistance with everyday activities, like getting dressed or bathing. HCBS are often designed to enable people to stay in their homes, rather than moving to a facility for care.
HCBS Assessments Generally Include Three Main Components. In California, assessors for each HCBS program determine eligibility and conduct an assessment to determine the amount and type of services that a client may need from the particular program to remain safely in his/her home and community.
