2. Begin by entering your personal information. Fill in your name, UIN, email, and phone number in the designated fields.
3. Indicate the type of experience you are requesting an absence from by checking the appropriate box: Clerkship, Elective, or Sub-Internship.
4. Provide the title of your clinical experience and its location in the specified fields.
5. Enter the date(s) of your anticipated absence clearly in the provided section.
6. Select a reason for your absence from the list provided. Ensure you check only one option that applies to your situation.
7. If applicable, describe any other circumstances that may warrant an absence in the space provided at the end of the reasons section.
8. Once completed, save your form and email it to your Clerkship/Elective/Sub-Internship Director for review as instructed.
